grooming, mobility assistance.
• Alzheimer’s and Dementia Home Care –
Assistance with activities of daily living, safety
assistance, managing behaviors and family
support and encouragement though
engagement.
Who pays for it:
Personal Funds, Some Long-Term Care
Policies, Some Assistance offered through
Veterans Affairs office.
HOME HEALTHCARE SERVICES
Home Health Care Services Offer:
• Help with Recovery from illness, injury,
surgery, and long-term conditions.
• Skilled Nursing
• Home Health Aids
• Physical Therapy
• Occupational Therapy
• Speech Therapy
• Social Worker
Who pays for it:
Medicare, Medicaid, Private Insurance,
Personal Funds
HOSPICE SERVICES
Hospice Services Offer:
• A team of Healthcare Professionals including:
Nurses, CNAs, Social Workers, Clergy,
Trained Volunteers, Therapy Services
• Pain Management
• Assistance with emotional, psychosocial and
spiritual aspects of the dying process.
• Provide: Medication, Medical Supplies, and
Special Equipment
• Family Education on how to care for their
loved one
• Bereavement care and counseling for family
and friends after someone has passed.
Who pays for it:
Medicare, Medicaid, Non- Profit Funds, and
some Private Insurance Plans.
