Enroll no later than two weeks prior to your benefits start date.

Call 866-460-0515, Monday-Friday, 8:00 am – 5:00 pm CST
You have 31 days from the start date of coverage to make changes.
After that, you are locked in to your selections until the next Open
Enrollment period unless you have a Qualifying Event. If you have
a Qualifying Event, you have 31 days from the event date to make
changes.
Qualifying Events include:
• Marriage
• Divorce
• Having a baby
• Adopting a child
• The death of a spouse or a child
• Loss or gain of coverage that was not previously available to you
• A status change
• A leave of absence
• National Medical Support Notice or other legal decree
• Online: electbenefits.com/pilot (Select Qualifying Event Link)
• By phone: 866-460-0515 (Select Qualifying Event option)
You will be eligible to enroll in all benefits once you are full-time. The
effective date of coverage will be the date of your status change. You
must contact the Benefits Department within 31 days from the effective
date to enroll in your new benefits.
This is the amount that was deposited into your 401(k) by PFJ for the
company match. It is reported under deductions, but it is not deducted
from your pay.

In the email, be sure to include: your new address, your employee
number and the name of the county you live in
Your benefits cancel on the last day of the pay period during which your
employment ends.
COBRA allows you to extend your medical, vision and dental coverage
for up to 18 months after separation from Pilot Flying J. COBRA
coverage is more expensive than coverage as a PFJ Team Member since
you pay the entire premium.
You will receive a packet through the mail from UHC Services with
instructions on how to enroll. You can also go to uhcservices.com or
call 866-747-0048 to enroll in COBRA.
