TO IMPROVE YOUR
DOCTOR VISITS
Write a list of questions for your doctor.
Keep notes on any new symptoms you have been experiencing since your last visit.
Gather all items you need for your appointment, which may include: a list of current
medicines (including over the counter medicines), insurance cards, healthcare
directives, hospital discharge papers (if you have recently been hospitalized), any
medical records or test results you have been given by other health care providers,
and a list of any medical equipment you currently have or may need.
Don’t be afraid to speak up. Tell your doctor what you think he or she needs to know.
Tell your doctor personal information, even if it makes you uncomfortable.
Have a notepad and pen handy and take notes during the appointment.
Contact your Health Coach and let them know how the appointment went.
Update your calendar with dates of any new appointments.
Get your new prescriptions filled, if any, and update your personal medication list.
Call you doctor for follow up if your symptoms get worse, you have problems with
your medicine, you have not heard back from test results as expected, or if you have
any questions that were not answered.
