WHAT ARE INTEGRATED SENIOR
CARE MEDICAL PRACTICES?
As the aging population grows, health care
providers are developing ways to coordinate
care, lower costs, and improve outcomes. An
integrated care medical practice considers a
patient’s unique needs and uses a personal, coordinated
approach to meet them. The patient
has access to various services, programs, and
resources, all under one roof, which may include
anything from primary care or treatment
for a specific condition, to an onsite wellness
center and fitness and nutrition classes.
The focus in the integrated health care
model is on treating the whole person. The
idea is to create an “one stop shop” for older
adults to manage not only their health, but also
their wellness and quality of life. An emphasis
is placed on really getting to know the patient
and creating a plan to meet their needs in every
area of life.
Services offered vary by practice and may
be in-person and/or virtual. Services offered
may include:
• Transportation
• Fitness classes
• Educational classes and programs
• Spa services like therapeutic massage
• Weight management
• Telemedicine
• Personal health navigators to assist with
insurance and Medicare issues
• Assistance with state benefit programs such
as food stamps, Medicaid and Medicare
