Qualify for the Wellness rate when you and your covered spouse,
if applicable, complete both steps:
1. Download and log in to the Fuel4Life app.
2. Register for Teladoc and complete your medical history.
Go to Teladoc.com.
You can qualify for the Wellness rate anytime during the year. Please
allow 2-4 weeks for the rate to take effect.
Your benefits cancel on the last day of the pay period during which
your employment ends. Most benefits will end if your status changes
from full-time to part-time. Affected benefits will end on the date of the
status change.
If you switch to part-time status and you were enrolled in a health plan,
you will have the option to continue that medical coverage until the end
of the calendar year.
COBRA allows you to extend your medical, vision and dental coverage
for up to 18 months after separation from Pilot Flying J. COBRA
coverage is more expensive than coverage as a PFJ Team Member since
you pay the entire premium.
You will receive a packet through the mail from UHC Services with
instructions on how to enroll. You can also go to uhcservices.com or
call 866-747-0048 to enroll in COBRA.
This is the amount that was deposited into your 401(k) by PFJ for the
company match. It is reported under deductions, but it is not deducted
from your pay.

In the email, be sure to include: your new address, your employee
number and the name of the county you live in.
• Company-paid benefits and 401(k) roll over year to year.
• Medical, Dental, Vision, FSA, Team Member-paid Life and Disability
end on 12/31. To add or continue benefits coverage, you must make
elections each year during Open Enrollment.
• Healthcare FSA rolls over up to $500.
• If you re-enroll in a PFJ health plan, any remaining HRA funds roll over.
