COBRA Information
What is COBRA?
COBRA allows Team Members to extend their Pilot Flying J (PFJ) Medical,
Vision or Dental coverage(s) for up to 18 months after separation from PFJ.
When do PFJ benefits end?
When a Team Member is no longer employed with PFJ, their Medical,
Vision or Dental coverage(s) end on the last day of the pay period for
which they worked. This includes access to any remaining HRA or FSA
funds, unless COBRA is elected.
How will Team Members know when to enroll?
United Healthcare (UHC) Benefit Services will mail a packet to the Team
Member with instructions on how to enroll after the Team Member’s
separation date is processed. However, Team Members can enroll before
the packet arrives by visiting UHCservices.com or by calling 866-747-
0048.
All COBRA billing, enrollment and cancellation notices are sent from UHC
Benefit Services to the home address on file with PFJ.
How much does COBRA cost?
COBRA rates will be included in the packet from UHC Benefit Services.
How long do Team Members have to enroll in COBRA?
Separated Team Members have a 60-day eligibility period to enroll in
COBRA benefits. The eligibility period begins on the date shown on the
Qualifying Event Notification letter from UHC Benefit Services.
When are COBRA premium payments due?
Payment is due within 45 days of enrolling and can be made by mail,
online at UHCservices.com or by calling 866-747-0048.
When does COBRA coverage go into effect?
COBRA is backdated to the date that a Team Member’s coverage ended. If
a Team Member elects to enroll in COBRA on the last day of their eligibility
period and pays for their first month of premiums, any covered claims
from the date of their separation will be covered.
