Absences will not be changed from unexcused to excused educational enhancement opportunity days until educational proof of trip has
been submitted to the Principal. Educational proof must be an essay, PowerPoint, etc. to show educational significance of the trip/absences.
Principal or designee will submit a copy of the completed and signed educational enhancement opportunity application and educational
proof to the Director of Pupil Personnel office. The Director of Pupil Personnel will then change the absences from unexcused to excused.

UNEXCUSED ABSENCES
All other absences will be considered unexcused.
NOTIFICATION REQUIRED
Parents/guardians are requested to notify the school by phone of the student’s reason for absence.
Written verification of all full day or partial day absences is required within five (5) days upon the student’s return to school.
Five (5) days due to student illness may be excused by written parental notes within the school year. However, if the
Principal/designee is presented with what appears to be a fraudulent excuse, the parent/student may lose their right to submit additional
parental notes.
During the school year, ten (10) days may be excused for medical reasons, which include but are not limited to,
physician, dental or eye care visits. After the tenth (10th) medical excuse has been submitted, a conference may be requested
by the Principal, Director of Pupil Personnel or an attendance officer. If any medical excuses or documentation are found to
be altered or verified as fraudulent, any future medical documentation may be required to be sent directly from the medical
facility to the school, which could lead to the absences being counted as unexcused.
Exception: Parents/guardians of chronically ill students or students with disabilities should notify the Principal in writing
in order for those students to receive special consideration concerning the 10-day limitation.
If a student is unable to attend school for medical reasons for more than five (5) consecutive school days, the
parent/guardian shall be contacted and asked to notify the Director of Pupil Personnel to request a Home Hospital application.
Upon presentation of a completed application, the Home Hospital Committee will approve or deny the application based on state
criteria.
MAKE-UP WORK
Students are expected to accept responsibility for make-up work missed for excused absences.
The number of days allowed to complete the work will be equal to the number of days missed.
Students are not entitled to make up daily work missed for absences because of suspension from school but are allowed to do the
following:
1. Take major tests, such as final examinations or unit tests, for credit upon returning from a suspension, and
2. Hand in for credit major projects or term papers assigned before the period of suspension but due during the time the
suspension occurred.
Work assigned and due during the suspension will not be accepted for credit.

excuse form provided by the school. The medical excuse form must be completed by a physician. All court ordered and CDW
students must present a medical excuse form for every medical excuse.
When a student has accumulated six (6) unexcused absences and/or twenty (20) unexcused tardies, he/she will not be allowed
to participate in any school activities (including, but not limited to, athletics, extracurricular field trips, dance, cheer, prom, and
other club and organization trips that are not assessed.)
Upon the student’s return to school after the sixth (6) unexcused absence or twentieth (20) unexcused tardy, the student may
request a good effort contract stating that upon no further unexcused absences or tardies, the student will be allowed to participate in
school activities. The contract should be requested, signed, and submitted within five (5) days. If the student accumulates one
additional unexcused absence or tardy before or after the contract is signed, he or she will forfeit participation in school activities
for the remainder of the school year.
The district may review the excuses and any supporting documentation/information as submitted and may take steps to
address attendance issues, including but not limited to: requiring more extensive documentation/information in support of future
excused absences, exercising interventions/corrections regarding any absences determined to be supported by incorrect or
