committed to developing innovative
services that meet the needs of our Tampa
Bay community. This fall, we will bring
the future of HIV care to a new level
with the opening of our region’s first-ofits
kind medical home serving those with
HIV – HOME 3050.
HOME 3050 is a partnership of Empath
Health’s AIDS Service Association of
Pinellas (ASAP) and BayCare Health
System’s Pinellas Care Clinic (PCC),
two leading nonprofit HIV service and
care providers. We are combining our
longtime expertise to deliver fullservice
health care and social services
to participants all on one convenient
campus, located at 3050 1st Avenue
South in St. Petersburg.
At HOME 3050, our personalized care
teams will provide the highest level
of patient-centered care and services with
compassion, dignity and respect. From
the start of intake, everything will be
coordinated to meet the individualized
needs of those we serve. Our goal is to
achieve seamless care experiences, HIV
this one-stop location for comprehensive,
high-quality health care and social
services to the community at large, so all
those in need can access this kind of
expertise and experience.
The ongoing support from our
community is ever so vital to our mission
of care. We are deeply grateful for our
community donors and volunteers who
are supporting the development and
work of HOME 3050. Together, we can
make a difference and improve the many
lives impacted by HIV.

quality of life for every patient and client.
HOME 3050 will offer primary and
HIV specialty care, laboratory, pharmacy
and benefits services, counseling, medical
case management, a food and personal
needs pantry, HIV and STD testing,
therapeutic and support groups, ‘Ryan
White’ program eligibility determination,
and education that focuses on prevention
and wellness.
The pharmacy services, provided by
Empath Health, will benefit HOME 3050
participants and beyond. All of the
pharmacy’s proceeds will be reinvested
into our community HIV services.
It is an honor to partner with PCC and
our mutual commitment remains strong
to help those with HIV/AIDS live even
healthier lives through HOME 3050. In
the future, we look forward to offering
