Medicaid
Medicaid is a program for individuals and families, which pays for
medical services provided by health care professionals who have entered
into a participation agreement with the Department for Medicaid
Services. An application for Medical Assistance may be filed at the
local Department for Community Based Services Office. At the time of
application, an individual or family member should be advised to bring
proof of:
• Social Security Number
• Health Insurance
• Pregnancy (Doctor Statement verifying
pregnancy and expected due date of delivery), if applicable.

• Non-Pregnant parents should bring proof of resources (last 3 months
bank statements from checking or savings account, life insurance
policies, stocks and bonds, etc.), if they want Medicaid coverage for
themselves.

Medicare
The Medicare health insurance program provides acute care coverage
for persons receiving either Social Security or Rail Retirement benefits.
Traditionally, those receiving Medicare are age 65 or older. Persons
who qualify for Social Security Disability Insurance (SSDI) must wait
twenty-four (24) months after being qualified for SSDI to start receiving
Medicare benefits. To receive medical coverage until Medicare
begins; the Social Security office must be contacted and
after explaining the need, apply for Medicaid coverage until
Medicare coverage begins.
Hospital Insurance (Part A): The basic hospital insurance
program covered services for hospital.
Medical Insurance (Part B): Elderly persons can receive
benefits under this supplementary program only if they sign up
for them and agree to a monthly premium.

KENTUCKY PROGRAMS FOR PERSONS
WITH BRAIN INJURY
Note: None of these programs provide any direct financial
assistance. They do, however, provide qualified people with the
ability to purchase needed services that will enable them to live
in the community.
Acquired brain Injury Medicaid Waivers: Acute and Long-Term
The ABI Waiver program is a community re-entry program for
rehabilitation and training during a period of transition. It is
not a life-long service. There is, however, a Long-Term Waiver
option for those who may need it.
The definition of Acquired Brain Injury is: an injury to the
brain which is not hereditary, congenital or degenerative.
Acquired Brain Injury includes central nervous system injury
from physical traumas, from anoxic or hypoxic episodes
and allergic conditions, toxic substances, and other acute
medical/clinical incidents. Not included in this are: strokes
treatable in nursing facilities providing routine rehabilitation
services, spinal cord injuries in which there are no known or
obvious injuries to the intracranial central nervous system,
progressive dementia, depression and psychiatric disorders,
mental retardation and other birth defect related disorders
and conditions which cause the individual to pose an
unmanageable level of danger to the community.
Who is medically eligible?
Persons between the ages of 21 and 65 years of age, meet
nursing facility level of care, have a primary diagnosis of an
acquired brain injury and have the potential for rehabilitation
and reintegration into the community.
