evaluation and application for this program.
Personal Care Attendant Program
This program serves adults with severe physical disabilities that
are at risk of being institutionalized and allows them to live in
their own homes and communities.
• Eligibility
- Adults over 18 years old with a functional loss
of two or more limbs and the ability to hire/
supervise an attendant are eligible for this program.

- Other day-to-day activities
State Health Insurance Assistance
Navigating Medicare and Medicaid insurance issues can be
very confusing. The State Health Insurance Assistance or SHIP
program provides you with free counseling in making these
decisions.
Kentucky SHIP:
• Educates seniors/persons with disabilities on health
insurance coverage, benefits and consumer rights.
• Provides assistance and education on a one-on-one basis
or through educational forums.
• Protects consumers against fraud or misdirected
collections.
• Empowers consumers to make informed health
insurance choices.

Coordinator.
TRAUMATIC BRAIN INJURY BEHAVIORAL PROGRAM*
The Traumatic Brain Injury Behavioral Program establishes
identification of those affected by traumatic brain injury who
are in need of behavioral services. The TBI Behavioral Program
will provide services through crisis intervention, residential,
targeted case management and wrap-around services. For

Who is eligible for the TBI Trust Fund?
This state-run program provides temporary financial assistance to
qualified persons with help funding goods or services they need
because of a brain injury. This program is open to all Kentucky
residents, who have a medically documented brain injury. This
program is open to qualified persons of all ages regardless of
income levels. The applicant only needs to show the need and that
all other avenues for support have been exhausted.
Not eligible for the TBI Trust Fund are:
• Individuals treated in a nursing home
• Spinal cord injuries
• Progressive dementia
• Psychiatric disorders
• Intellectual disability, birth defects, or congenital defects
• Neurological conditions of a degenerative or progressive
nature
What services does the TBI Trust Fund provide?
• Case management
• Environmental modifications
• Companion or personal care
• Therapies and counseling services
• Respite care
• Structured day programs
• Wrap around services and supports (other services and
supports that aren’t listed above)
What services are NOT covered?
• Medications
• Institutionalization
• Hospitalization
• Legal/Attorney fees
• Any previous incurred debt or bill
How to Apply for the Trust Fund
Call toll free 1-855-816-9577 and request an application. To
assist you in the application process, please have the following
information ready when you call:
• The name of the person with the brain injury
• Their Social Security Number
• Date of Birth,
• Date of Injury
• Name of Hospitals that provided treatment
• Medicaid/Medicare numbers, if you have them
• Contact information
Contacting Your Local Area Agency on Aging
If you are interested in applying for any of the services not
offered by the Traumatic Brain Injury Program of KY DAIL,
you will want to contact the Area Agency on Aging in your
region. You local Area Agency on Aging can help you apply
for the Michelle P., Home and Community Based Waiver, and
the Supports for Community Living Medicaid waivers. The
